High Order Aberrations (HOA) Related To Refractive Surgery/LASIK

High order aberrations (HOA) refer to complex imperfections of the optical system and are normally present in all eyes in minor unnoticeable amounts. However as a result of optical refractive surgeries such as LASIK, unnatural and irregular shape of the cornea can result in increase in HOA and thus result in halos, double vision, smears and other visual disturbances.

High order aberrations are measured using a wavefront aberrometer. The wavefront aberrometer measures the distortions of light as they pass through the patient’s cornea.

Dry eyes are a common complication of LASIK procedure. Although in most cases the post lasik dry eyes is temporary, however some patients might continue having dry eyes for years after the procedure. Most of these patients are those who had some level dry eye before the surgery too.

Rigid gas permeable contact lenses don’t come in contact with the corneal surface e and thus are helpful in solving the vision issues due to irregular cornea.

Scleral Contact lenses

Scleral contact lenses don’t rest on the corneal surface, rather they rest on the sclera due to their larger diameter. They’re gas permeable therefore corneal hypoxia is prevented.

Scleral contact lenses are helpful in solving various corneal surface related vision issues and also dry eyes or other issues due to corneal irritation with conventional lenses.

Lasik (Laser-Assisted in situ Keratomileusis), a type of eye surgery for correction of also errors of refraction is also known as laser eye surgery or laser vision correction.

LASIK provides a permanent solution for errors of refraction in most patients, getting rid of eye glasses or contact lenses. However, there are certain complications associated with the procedure too and should be known by the patients before getting LASIK procedure done.

Correction errors

Summary of the findings:

  • LASIK Dry Eye: Up to 30 % of the subjects developed new dry eye symptoms.
  • LASIK Symptoms: 45% of the subjects, who were symptom free prior to LASIK, developed symptoms post LASIK. (halos, starbursts, glare, and ghosting)
  • LASIK Dissatisfaction: Up to 4 % of the subjects were dissatisfied with their vision post LASIK.
  • LASIK Dissatisfaction: Up to 1% of the subjects visual symptoms caused extreme difficulty to do usual activities.

Based on these results the FDA stated “Given the large number of patients undergoing LASIK annually, dissatisfaction and disabling symptoms may occur in a significant number of patients.”
